Supported Living Services (SLS)
Supported Living Services (SLS) consist of a broad range of services to adults with developmental disabilities who, through the Individual Program Plan (IPP) process, choose to live in homes or apartments they themselves lease or occupy in the community. FACT often provides SLS to clients who reside at home with their families but who are preparing to move out on their own. SLS can help with common daily living activities and emergencies, becoming a participating member in community life, managing personal financial affairs, selecting and moving into an apartment or home, choosing personal attendants and housemates, as well as other supports.
SLS services help individuals exercise meaningful choice and control in their daily lives, including where and with whom to live. SLS is designed to foster individuals’ nurturing relationships, full membership in the community, and work toward their long-range personal goals. Because these may be life-long concerns, SLS are offered for as long and as often as needed, with the flexibility required to meet a person’s changing needs over time, and without regard solely to the level of disability. FACT’s goal through its Supported Living Services is to support each individual to establish and maintain a safe, stable, and independent life in his or her own home.
Specific services provided through SLS cover a wide range of support needs, including:
- Autonomy and Learning to Reside Independently
- Health and Medical Needs
- Lifelong Learning
- Mobility Training
- Money Management and Budgeting
- Social/Emotional Recreational ComponentWork/Vocation

Living independently or planning to live independently within the next two years
Expressed desire to participate in Supported Living Services
*Diagnosed with ASD, TBI, ID, DD, CP, and/or other developmental disability as the primary diagnosis
Services are flexible and tailored to clients’ needs and preferences, occur in the individual’s home community (including work and school settings), and are scheduled at the convenience of the individual.
