Supported Living Services

FACT’s Supported Living Services (SLS) help adults with developmental disabilities live independently in homes or apartments. We assist those who live with their families but are preparing to move out. Services include daily living activities, emergency support, community participation, financial management, and choosing personal attendants and housemates.
SLS helps individuals make choices about their daily lives, build relationships, join the community, and achieve long-term goals. These services are flexible and provided as long as needed, regardless of the level of disability. Our goal is to help each person establish and maintain a safe, stable, and independent life in their own home.
Specific services include:
- Learning to live independently
- Health and medical support
- Hygiene assistance
- Lifelong learning opportunities
- Mobility training
- Money management and budgeting
- Nutrition guidance
- Safety planning
- Social and recreational activities
- Work and vocational support
FACT is vendored with Westside Regional Center for this service and able to provide up to 16 hours of support a week.
