FACT offers a range of therapeutic supports and programs that focus on improving well-being, daily functioning, and overall life-satisfaction through a holistic and person centered approach. Whether it’s one on one therapy, or getting out into the community to reach one’s goals, FACT offers clients a robust variety of services that best fit the client’s needs. All services are offered through Self determination, traditional, and Private Pay

SUPPORTED LIVING SERVICES (SLS)

Supported Living Services (SLS) consist of a broad range of services to adults with developmental disabilities who, through the Individual Program Plan (IPP) process, choose to live in homes or apartments they themselves lease or occupy in the community. FACT often provides SLS to clients who reside at home with their families but who are preparing to move out on their own. SLS can help with common daily living activities and emergencies, becoming a participating member in community life, managing personal financial affairs, selecting and moving into an apartment or home, choosing personal attendants and housemates, as well as other supports.

SLS services help individuals exercise meaningful choice and control in their daily lives, including where and with whom to live. SLS is designed to foster individuals’ nurturing relationships, full membership in the community, and work toward their long-range personal goals. Because these may be life-long concerns, SLS are offered for as long and as often as needed, with the flexibility required to meet a person’s changing needs over time, and without regard solely to the level of disability. FACT’s goal through its Supported Living Services is to support each individual to establish and maintain a safe, stable, and independent life in his or her own home.

GAP PROGRAM

Generating All Possibilities (GAP) helps young adults 18+ to transition into adult life. GAP is hosted in two primary locations: GAP Westside and GAP Valley.

The program includes support on college campuses, internships and volunteer opportunities with community partners, and a wide selection of workshops and activities designed to spark passion and bolster independence. Participants in GAP select from our menu of options to create individual schedules comprised of activities that match their goals and interests. Many young adults attend college one to four days a week and participate in GAP’s community programs on other days of the week. Young adults who have either completed college or are not interested in taking classes participate in the community programs up to five days a week.

Self-Employment Microenterprise
& Competitive Integrated Employment

FACT works with each individual for at least six months to help get the business get started. Before starting a business FACT provides assistance with the following:

  • Understand business basics
  • Determine a sellable product/service
  • Clarify if the business idea is something the individual enjoys
  • Write a plan for the business
  • Analyze costs to run the business
  • Think about who will help run the business
  • Plan how the business will affect benefits like SSI

Individual Therapy

Individual psychotherapy allows us to develop and strengthen coping skills while gaining insight into ourselves and our patterns of behavior. It can serve as a catalyst for change and a tool for promoting growth, while providing much needed support during difficult and trying times. Our therapists rely on strength based, client centered approaches that view the client as the expert of their own experience while working alongside clients to achieve greater satisfaction and quality of life.

Parent Education and Support Group

Parent groups meet at the same time as the social skills and support groups to provide parents with ongoing support and strategies to assist their child in social situations, manage difficult behaviors, and navigate successfully through developmental stages. The parent group curriculum includes a mix of articles, books, videos, parenting exercises and more.

Young Adult Support Group

Ages 18+ This group focuses on the transition into and throughout adulthood.

Core themes build on adapting fundamental social behaviors and applying them to adulthood, including identifying and exhibiting socially appropriate behaviors in areas including (but not limited to) conflict resolution, self-advocacy and self-expression, peer relationships, dating, and internet behavior. The group also works on pro-social coping skills to help members successfully deal with many of the inherent challenges that face this age-group. The curriculum is adapted each week based on the current needs and experiences of the members.

In-School Support

FACT provides classroom support for grades K-12.  Our Behavior Intervention Implementors, (BII’s) and our Behavior Intervention Developers (BIDs), work with your child to help maximize their in-class learning.  Participation is the exclusive decision of the administration of the child’s school.  You may contact your child’s school administration to see if FACT’s services are available.
